The rehabilitation continuum is made up of a progression of health care services across several different care settings. It includes acute care hospitals, acute inpatient rehabilitation, transitional care units in skilled nursing, outpatient rehabilitation and home health care.
St. Luke's provides specialized therapy services at Providence Sacred Heart Medical Center & Children's Hospital, Holy Family Hospital, and St. Joseph Care Center. With its comprehensive therapy staff, St. Luke's also provides contracted therapy staffing services to Providence VNA Home Health and rural hospitals throughout the region. This collaboration is creating an even stronger, integrated and connected rehabilitation continuum of care for patients from hospital discharge to getting back to their community.
